The model is designed as a continuous community-to-evidence-to-action cycle.

Who is being missed?

What barrier explains the gap?

What is the appropriate intervention?

Did the person receive the service?

Did it work and what should change?

A person may be identified in the community but fail to reach appropriate care. MCH Connect focuses on navigation and linkage.

Repeated barriers can be documented through MCH Intelligence and translated into targeted Community MCH Action.

Implementation is followed by evaluation, learning and adaptation rather than assuming success.
