A community-to-health-system continuity organization.
The proposed organization is designed to connect underserved mothers, newborns, children and adolescents to care, evidence and action.
Don’t just count the people we reached. Know what happened next.
The organization will serve underserved communities in Busoga from the community health-system interface.
Identify underserved families; understand barriers; connect them to appropriate services; support navigation and referral; follow up after contact with the health system; generate evidence on unmet needs; and work with communities, facilities, local governments and partners to develop targeted solutions.
Vision
A Busoga where every mother and child, regardless of where they live or socioeconomic circumstances, can access, navigate and continue receiving the care they need to survive and thrive.
Mission
To improve maternal, newborn, child and adolescent health among underserved communities in Busoga by connecting families to appropriate services, strengthening community capacity, supporting continuity of care, generating evidence on unmet needs, and working with health systems and partners to address barriers to care.
Not a parallel health system.
The organization is intended to function as a connector and evidence-to-action platform within an existing ecosystem of communities, facilities, government, universities and partners.
Community
Listen to families and understand the barriers that shape care-seeking and continuity.
Connection
Help people navigate services, referrals and follow-up without replacing professional care.
Evidence
Document unmet needs and generate research, monitoring and learning that can guide decisions.
Action
Translate evidence into targeted interventions with communities and health systems.
